Someone there as you settle back in.
Someone there when you get home: the kettle on, the shop unpacked, a meal eaten together, and company to the follow-up appointment. Planned before you leave the ward.

Coming home is a change of pace
After a busy ward, home can feel very quiet. There may be shopping to organise, meals to think about and appointments to get to. Family and friends may help, but cannot always stay as long as they would like.
Whitegreen can provide companionship and light practical support alongside the discharge arrangements agreed with your hospital team. Tell us what your days at home will involve and where company would help.
The first visit home
Your companion arrives at the agreed time and starts by asking how you want to spend the visit. That might mean putting the kettle on, unpacking a small shop, preparing a simple lunch and sitting down together for a proper conversation. We can help write a shopping list, put everyday items within easy reach and talk through the practical plans for tomorrow.
We take account of the instructions in your discharge plan. You set the pace. There is no need to fill the visit with activities when what you want is someone to sit with.
The rest of the first week
On the next visit, your companion can collect the agreed groceries, prepare another simple meal or help with light household tasks. There is time to chat, watch something together or return to a hobby you have missed.
When you are ready and it fits your discharge advice, we can join you for a short walk or a trip to the shops. For a follow-up appointment, we plan the journey, accompany you, wait with you and help you get home. We agree transport, timing and any extra costs beforehand.
Your companion writes a note after every visit, shared with the person you nominate. Urgent concerns are a phone call. As the week goes on, we talk with you about the company and practical help you want next. These are examples to build your own visit plan around, rather than a fixed first-week package.
Plan it before the journey home
You or someone helping you can contact us with your postcode, expected return date and the support you want. Let us know about any care already arranged and any help needed during the visit. We discuss suitability, confirm available times and agree the cost before booking.
Hospital plans can change. Keep us updated so that the visit arrangements follow the agreed discharge date.
Working alongside the discharge plan
The hospital team decides when discharge is appropriate and what care is needed. Whitegreen provides companionship and practical everyday support. We do not replace personal care, nursing, rehabilitation or any supervision the discharge plan requires.
If you need several kinds of support, we can discuss where companionship fits and who will be responsible for the other parts. Explore companionship alongside care.
A visit that fits the day
Tell us whether you want company for part of the day, a longer visit or an overnight arrangement. We agree the hours and support rather than fitting every enquiry into the same visit length.
For hospital and community teams
We welcome enquiries about companionship for adults returning home. With the person's consent, share the purpose of the support, the proposed location and timing, and the arrangements already in place. We will discuss the proposed role and confirm what can be provided. Professional referrals.
